  4. Statement of Good Faith: A declaration that you possess a good faith belief that the disputed use of the material is not authorized by the copyright owner, its legal agent, or statutory fair use principles.
  5. Statement under Penalty of Perjury: A statement, made under penalty of perjury, that the information provided in your notice is accurate and that you are the copyright owner or authorized to act on behalf of the copyright owner.
  6. Signature: A physical or verified electronic signature of the copyright owner or authorized representative.
